After you fill out the form, e-mail the form's contents by clicking on the Submit by Email link on the top right corner. Follow the instructions adobe gives you.

Start by Saving the file somewhere you can find it (e.g. desktop or documents folder), then e-mail the file to cconrad@email.arizona.edu. Pay for your registration below.
- Print a copy of the document for your records. You will be notified when your application has been processed.
