2015-16 Pro Chapter Annual Report Guidelines and Submission Form

Due: May 30, 2016 | Covers period from May 31, 2015 to April 30, 2016


Offline/email alternative

As an alternative, you also may download this annual reports template file [Microsoft Word .doc format, compressed into a ZIP file], fill it out offline, and submit it via email. Because the annual reports form is a long one, you may prefer to do it this way so that you can fill it out pieces at a time and eliminate any likelihood of connection errors.

Annual reports — required for all pro and student chapters — are due May 30, 2016.

Regional directors use these reports to pick Chapter of the Year, First Amendment, Diversity, Chapter Communications, Campus Relations and Professional Development awards.

Please use no more than 125 words to summarize each program and activity and no more than 250 words in the “Additional Comments” section, if desired. Links to coverage or publicity about events may be added to summaries. Chapters aren't required to have programs in every category, but it's encouraged.

All annual reports should be submitted online (the form appears below) or via e-mail using the Word document (blue box, right). If you have questions about the report or cannot meet the filing deadling, contact Tara Puckey via email or by phone at 317-920-4784, or contact your regional director.
