Adam L Scheffler is an independent health care journalist, health policy analyst, and licensed social worker. He most recently freelanced as a copy editor and correspondent at American Medical News. From 2002 through 2006, he served on the Steering Committee of the Chicago Patient Safety Forum (now the Coalition for Quality and Patient Safety of Chicagoland). He wrote a series about online patient safety resources for Quality and Safety in Health Care, a BMJ Group journal, in 2002 and 2003. He was the lead author of "A Collection of Definitions," reprinted in Medical Error: What Do We Know? What Do We Do? (Jossey-Bass, 2002) and originally published in Lessons in Patient Safety (National Patient Safety Foundation, 2001). He was the lead editor of Proceedings of Enhancing Patient Safety and Reducing Errors in Health Care (National Patient Safety Foundation, 1999). He also has worked as a broadcast journalist in Chicago, New York, Pittsburgh, Atlanta and Washington, DC.
